Welcome to our Scholarship Forms Hub! It is important to keep your contact and school information up to date in order to receive your scholarships on time. Please select the applicable form, complete, and return to KEF at [email protected]. Please read the descriptions below to ensure you’re downloading the correct form for your needs.

  • Koniag Descendant Application Form: This form is designed for students who are not yet registered as a Koniag Descendant. Please complete and submit the application form to Koniag Shareholder Services with the necessary information to be considered for our scholarship programs. Your application will remain pending until KEF receives your enrollment confirmation from you via email.

  • Change of Address Form: Have you recently moved or changed your contact information? It’s crucial to keep your records up-to-date to ensure you receive important communications regarding your scholarship. Please fill out the Address Update Form with your current contact details.

  • Change (or Selection) of College Form: If you’ve decided to transfer to a different college or university, it’s essential to inform us about your decision. Complete the Change of College Form to ensure your scholarship funds are redirected to your new institution. This form is also for students who had not yet decided which school they were going to attend when they applied for the KEF General, and have now made a decision, simply leave the “previous school” section blank.

  • KEF Standardized GPA Form: Our scholarship program requires students to maintain a minimum GPA to remain eligible for funding. The KEF Standardized GPA Form helps us track your academic progress and verify that you meet our eligibility criteria. Please submit this form along with your official transcripts.

  • FERPA Consent for Release: The Family Educational Rights and Privacy Act (FERPA) protects your educational records’ privacy. If you’d like to authorize KEF to speak to a family member regarding your scholarship and/or academic progress, please complete the FERPA Consent for Release form.

  • Request for Graduation Stole: With a generous donation from Koniag, KEF is proud to celebrate student accomplishments by providing students with a stole to wear during their graduation ceremony. Simply complete and return this form to the Kodiak or Anchorage office, or email it to [email protected].

  • Career Development Application: If you’re a student with no internet access and seeking funds for professional development opportunities, this form is for you. Complete the KEF Career Development Application to apply for support in attending workshops, conferences, or other career-enhancing events.
